Patient Rights
The patient has the right to be treated with respect, consideration and dignity.
Patients will be provided with appropriate privacy.
When need arises, the center will make reasonable attempts for heath care providers and other staff to communicate in the language or manner primarily used by patients.
The patient has a right to be provided, to the degree known, complete information concerning their diagnosis, evaluation treatment and prognosis. When it is medically inadvisable to give such information to a patient, the information is provided to a person designated by the patient or to a legally authorize person.
Patients will be give the opportunity to participate in decisions involving their health care, except when such participation is contraindicated for medical reasons.
The patient has the right to make informed decisions regarding their care.
The patient has the right to receive care in a safe setting.
Patient questions will be answered regarding any and all services available at the center, including but not limited to provisions for after-hours and emergency care, fees for service, payments policies, advanced directives and the credentials of their health care providers.
The patient has the right to exercise his or her rights without being subjected to discrimination or reprisal.
The patient has the right to be free from all forms of abuse or harassment.
The patient has the right to exercise his or her rights without being subjected to discrimination or reprisal.
The patient has the right to voice grievances regarding treatment or care that is not furnished.
The patient has the right to know that all disclosures and records are confidential, and is given the opportunity to approve or refuse their release, except when release is required by law.
The patient has the right to examine and receive an explanation of their bill regardless of source of payment.
The patient has the right to refuse to participate in experimental research.
The patient has the right to refuse any treatment.
The patient has the right to be fully informed about a treatment or procedure and the expected outcome before it is performed.
Patients have the right to change providers if other qualified providers are available.
Patient Responsibilities
The patient must provide complete and accurate information to the best of his/her ability about his/her health, any medications taken, including over-the-counter products and dietary supplements, and any allergies or sensitivities.
The patient must follow the treatment plan prescribed by his/her provider and participate in his/her care.
The patient must provide a responsible adult to transport him/her home from the facility and remain with him/her for twenty-four (24) hours, if required by his/her provider.
The patient must inform his/her provider about any living will, medical power of attorney, or other directive that could affect his/her care.
The patient must accept personal financial responsibility for any charges not covered by his/her insurance.
The patient must be respectful of all health care providers and staff, as well as other patients.
